Person-centered planning involves the development
of a "toolbox" of methods and resources that enable
people to choose their own pathways to success; the planners simply
help them to figure out where they want to go and how best to get
there. Our goal is to integrate person-centered planning into transition
planning and services for individuals so that they might become
active participants in the shaping and realization of their future
throughout their life transitions. The person-centered approach
relies much less on the service system by organizing truly individualized,
natural, and creative supports to achieve meaningful goals based
on the individual's strengths and preferences. This approach creates
a team of people who know and care about the individual, who come
together to develop and share a dream for the person's future,
and who work together to organize and provide the supports necessary
to make that dream a reality. Instead of forcing inappropriate
services upon an individual, after the team has determined what
has to be achieved, available support services can be tailored
to achieve these goals. |
